In return for being provided access to the collections and facilities of The Children’s Museum of Indianapolis, I agree to abide by the “Conditions of Access” stated on the reverse of this form.
As a visitor to The Children’s Museum of Indianapolis Collections we require that you abide by the following conditions:
- No outside visitors who have not been approved and granted access should be brought into the Collections. I understand this agreement grants permission for “one time use” and only as specified on the first page of this agreement under “Intended Use.”
- I understand the permission to examine the Museum’s collections, if granted, does not include permission to publish the contents or any manuscript or any excerpt there-from, to reproduce artifacts, or to sell, copy or redistribute photographs.
- Photography of specimens is limited to scientific use only. No images may be used for commercial purposes without written consent. I understand that permission to publish, reproduce or otherwise use Museum materials is solely the right of The Children’s Museum of Indianapolis and possibly subject to fees.
- Specimens are not to be prepared or otherwise modified without written permission (this includes coating specimens for photography). If a specimen is damaged during study please advise the relevant collection management staff member immediately.
- Preparation, molding and casting of specimens is prohibited without authorization.
- Access to collections and archives is limited to the stated purpose of an investigator’s visit.
- No eating or drinking is allowed in collection areas. No food or drink should be brought into the collection areas.
- Visitor hours are from 9.30am until 4.30pm. Visitors should leave the collections promptly at 4.30pm. All specimens should be returned to their cabinets before you leave.
- Upon completion of this project, I agree to present the Museum with a complimentary copy of any publication which uses the images or objects under the terms of this agreement.
- Any publication, reproduction or other use shall contain the following official acknowledgement: “Courtesy of The Children’s Museum of Indianapolis.”
- The Museum makes no representation that it is the owner of the copyright or literary property in unpublished manuscripts, musical works, pictorial, graphic or sculptural works, photographs, audiovisual works or sound recordings.
- If you intend on using 3D capture techniques such as photogrammetry, please see the Imaging and Reproductions policies and form. I understand that if I intend to use such techniques that I must first gain permission form the Curator.
Failure to follow these guidelines will result in immediate expulsion from the Collections, and may result in forfeiture of future collection access or borrowing privileges.
